Recently had a case 21 year male with history of loss of libido, testosterone 5.7 nmol/L, LH 3.1, FSH 2.0, SHBG 2.0 nmol/L (ref range 20-55) and calculated free testoterone 0.22 (reference range (0.20-0.64). All results have been repeated and confirmed on samples taken early in the morning. Look for an explanation of the very low SHBG, no history of steroid use, normal LFT, HbA1c and thyroid function, lean body habit with no family history of diabetes, been through puberty normally. Genetic causes have been raised by the endocrinologists not sure how likely this is and if any body has any experience.
