At Colchester General Hospital,Essex we have been having discussions with the MDT (multidisciplinary team) for some while about a way forward for our MDT use of outcome measures. We have now developed a booklet of collated measures which may be used with elderly patients and since the staff are finding them useful more and more with stroke or other complicated rehab patients. The booklet is easy to access in the patients notes and is being used often by the team to inform discharge decisions at our ward rounds and MDT meetings. So far we have carried out one 6 week audit on its use.Results are very promising. The booklet includes: Elderly Mobility Score T.U.S.S.(Timed Unsupported Steady Stand) 180* turn Berg Balance Scale Rivermead Mobility Index Modified Barthel Mental Test Score Geriatric Depression Scale Speech and Language Therapy grid Nutrition Assessment Nursing Dependency Score All scales are supposedly standard, valid and reliable.
