Sorry---sent the email without the attached wp file

I know that there is rarely a new eval format.  But a grad student and I
tried to put our neuro eval, that we use for teaching neuro evaluations,
into a pithy 2 page eval.

What do you think?  Any:  suggestions, comments, recommendations, etc.
Is it useable in the clinic?
