Could this be Reflex Sympathetic Dystrophy?  Any time during the hemiplegia, 
patient could have injured her hand secondary to not being aware of where her 
hand was located (ie contact with a table) which could have precipitated 
edema in the hand and the progression of RSD.  Just my initial thought....

Awaiting expert commentary...
