Dear Dr Sood, The patient who is having acute pain in the hand can be due to numerous reasons. As there is radiological evidence of Hetrotropic ossification you have to stop Passive exercises. Please verify is there is Reflex Sympathetic Dystrophy(Shoulder hand Syndromme)Which is very common if the shoulder if not mobilised early after a stroke. There can even be DVT,so check for any swelling, warmth, tenderness.(Chances of DVT is there only if the Limb was left unattended.)There are chances of a Subluxated shoulder(Look for radiographic evidences). I hope this is helpfull in your management. All the Best... Jose.P.Thomas Physical Therapist Department of Neurology SCTIMST.Kerala.India. Voice:+91 471 446458.
