Interesting question. In no particular order:

1. Lack of understanding of what the website is *for* by staff at all
levels.
2. Difficulty in appreciating what can be done in the time available
with the budget available.
3. Obtaining good quality content.
4. Ensuring that different content may/need/must be included in the site
in a common format/display.
5. People not updating their content regularly.
6. Problems over design content (I want a bright red background, not the
nice easy on the eye light blue you've chosen).
7. Lack of continuity of people providing content - they move, other
people take their job and want to do things differently.
8. Lack of understanding that bigger graphics, more animation and lots
of java don't necessarily make a better page.

Well, thats 8 points, which I hope helps. I suppose to summarise them
into a smaller number, it comes down to the fact that people who *see*
webpages, but don't understand how they work are not necessarily the
best people to tell a webmanager how to do their job! ;)
