At the risk of keeping this thread going longer than it should, I had the
misfortune to be involved in an incident whilst an SHO in medicine.

A patient suffered a cardiac arrest in ITU, to which the team were called.
Shortly after leaving the unit, there was a further crash call bringing
everyone back.  It seemed like a crazy turn of fate, a really bad day, but
the truth was even worse.  When the unit's defibrillator was unplugged
from the wall to treat patient 1, patient 2's respirator was unplugged as
well.  Only after the first crisis was over did the staff realise there was a
second well under way.....
