Well having set this hare running I was asked to provide a summary of sorts. This is based on the various replies, plus a quick look on the ADS website for EUSs and HLCs (although not all of these are on ADS, and of those which are on not all record the dates when the projects were undertaken), plus SMR User Group Newsletter No 1 (1995). I’m sure that this record is only partial (and possibly not totally accurate).
First SMR to use GIS
North Yorkshire from 1982?
Then there are a slew of SMRs adopting GIS in the early 1990s (Nottinghamshire 1993, Hampshire?, Berkshire 1992, Hertfordshire 1992/3, Northants 1993/4), presumably when desktop GIS was becoming more freely available (in a number of cases the SMRs were part of tests for their authorities)
UADs and GIS
The first UAD was Cirencester, undertaken from 1990 but this didn’t use GIS; the Northampton UAD was using GIS from 1995 (it was integrated within the SMR); Winchester from 1997 
EUSs and GIS
The first EUS was the Central Marches Historic Towns Survey which comprised Hereford and Worcester, Shropshire and Herefordshire and commenced in 1992. This used a CAD system though. Gloucestershire EUS 1995-8 (including Cotswold and Avon), Essex EUS 1995
HLCs and GIS
Cornwall 1994 (according to ADS); Avon 1995-8; Cotswold 1997-9; Lancashire (published 2002); Hampshire ?
