Hello Mike, Rebecca and all,
Incidents and near-misses of all kinds in an organisation would be reported through appropriate channels. Health and safety.
Eg in my Trust we fill in online Datix forms, which are then investigated. No identifying details are put in the narrative, and the form includes reminders about that, but personal details are recorded in a confidential manner for investigation and action.

An incident of the kind that Rebecca asked about would definitely be Datixed or equivalent. Local Risk Management teams may have access from central reporting to anonymised information about any similar incidents elsewhere in the country.

Helen

Hi Rebecca,
If anybody did know of an incident, they'd be skating on thin ice in terms of patient confidentiality if they disclosed anything
Regards

Hi all,

Me again I'm afraid, and I'm not expecting anything back from this rather sensitive request.

I have been asked to conduct a search on patient notes mix-ups, owing to a recent incident with a poor outcome.
In your hospital, do you know of any incident which has been reported where a patients notes have been confused with another?

Again, not expecting anything back as this is not something hospitals general broadcast, but I said I'd ask the community!
