Hello Mike, Rebecca and all, Incidents and near-misses of all kinds in an organisation would be reported through appropriate channels. Health and safety. Eg in my Trust we fill in online Datix forms, which are then investigated. No identifying details are put in the narrative, and the form includes reminders about that, but personal details are recorded in a confidential manner for investigation and action. An incident of the kind that Rebecca asked about would definitely be Datixed or equivalent. Local Risk Management teams may have access from central reporting to anonymised information about any similar incidents elsewhere in the country.
