Whether you search with or without inverted commas, the results are identical; the term 'cat bite' now returns the correct topic at the top, and the 'cat scratch' topic is not even on the first two pages of results. If a search of 'cat bite' did at that time return this irrelevant topic, then UTD was at fault. The only point I was really making was the 'cautionary tale' one you underline. I haven't read the article on cat scratch fever but it's possible that these two words appear in that article and that's why it was retrieved. It's useful as a cautionary tale to anyone who thinks that searching Google is the answer to everything. You can search Google but you need to discard the irrelevant hits. The junior doctor in this case obviously did not check all results. It sounds, though we should not prejudge any investigations that may be taking place, as if there was an error in clerking the patient, or in recalling the clerking when the end-user searched UTD. I'd love to use this as an case study in how using a librarian saves lives, but I think it would be stretching the point. I'm sure our users would like to be reassured about this (as would we). I've just repeated the search and it seems to be fixed, so I can't see exactly what happened previously. I think the lesson as far as UTD is concerned in this report is 'searcher beware', which applies to all searching really, so we have to emphasise it to our users across the board, and not assume some products are infallible. It does make you wonder about fully integrating these products with the EPR.
