Dear all

I have been asked to search for best practice/guidance on how to share ‘lessons learned’ from incidents/events (or indeed good practice examples) around all areas of the Trust.  Of course our usual sources do not necessarily give the detail of how it is done, so if anyone is willing to share examples from their Trust or work place I would be very grateful.
