There are a few available online:
National Museums Wales seem to have a strategy rather than a policy (they are doing great work on this) https://museum.wales/about/policy/equality-diversity/
And the Tyne and Wear archive museums seems an interesting example to look at:

The Museums of Liverpool seem to be revising theirs at the moment but it is worth contacting them (although it seems their policy is more focused on internal processes: http://www.liverpoolmuseums.org.uk/about/corporate/policies/

One thing to consider is not only how to put the policy in place but also how to establish at the same time measurements of success i.e. how will know whether you are implementing the policy e.g. by capturing data on your visitors demographic data etc.
