Hi Andrew,

Do you specifically include a statement about why you need personal
information, how you will store and process it, when you will use it etc?
You really need that to comply with GDPR.

I'd also say don't start from the questions, but start from what you
want/need to know to help you improve what you offer. Ideally, fewer
questions are better.
