Hello all,
Joining the conversation late due to being out of the office for a few days.
Just want to flag up that the situation isn’t always libraries in NHS Trusts providing services to public health and/or GPs. I was based in primary care as a clinical librarian but the latest NHS upheaval meant that I was TUPE’d out of the NHS and into
the council with Public Health. CCG’s have a service level agreement for some things from us (I do individual funding requests, ATHENS management, and searches for them where they are working with public health). There are some librarians working in Public
Health who have the background, and are working as clinical librarians at times (despite their job titles!)
Thanks,
Lynne

Hi Emily,
We have a SLA with our local CCG here in West Sussex – I don’t think there is anywhere you can look for that information, other than asking each individual Library Service.
If I remember correctly an audit was performed some years ago for libraries that provide services to their local Public Health teams, but as far as I’m aware this information
has never been collected for CCGs.
Somebody please feel free to correct me if I’m wrong!
Kind regards,
Erica

There are just under 40,000 GPs in England (https://digital.nhs.uk/data-and-information/data-tools-and-services/data-services/general-practice-data-hub/workforce)
And there are 195 CCGs (https://www.nhscc.org/ccgs/) each with their own budget.
It would be really interesting to find out how many CCGs have SLAs with a health library service. I’ve had a quick look on the HEE website, but can’t find anything. Anyone have any ideas where to look?

Yes it absolutely is! I was just saying that to someone else. Maybe CILIP needs to talk to the NHS about access to medical librarians and library resources for everyone working in the NHS. As Rosemary Soper says
here in Wales we’re lucky that anyone working in the NHS can access our online resources.

Sarah

I know Keith mentioned an HLG response but is this something we need to ask CILIP to be more proactive in promoting? They are our professional body after all and perhaps we need to ask for the #milliondecision

Maybe something needs to be done about this? Not sure what, but it’s clearly a gap in the evidence based work ethic of the NHS.

I think the problem with GPs is that they don’t all have access to a health library service. We don’t have a service level agreement with our local CCG (we have tried, but they don’t want to pay for a library
service). So for some, there is no clear point of access to a health librarian. I know we’re not on our own with this, there are other Trusts where the situation is the same.
If you ask the question Where do GPs and even the specialists look for evidence around how to make a clinical decision?”
unfortunately there is no straightforward answer, as there isn’t parity of access across England. Not sure about Scotland / Wales / NI – Sarah, is this the situation in Wales?

If someone is happy to do a rapid response that would be great. I really think the BMJ needs to buck their ideas up about evidence based medicine and the place medical librarians play in that. Maybe a whole article
about medical librarians or some kind of permanent page all about medical librarians and evidence based medicine would be a good thing for them to do?

Hello Sarah,
Interesting… in a way this is a bit like the BMJ letter about the STRIDER trial, recently discussed on this list, in that it fails to appreciate the help that librarians can provide. Does this one need
a rapid response too?
Best wishes,

Hello,
Not sure if this has already been discussed but I’ve just been made aware of the following article:
https://www.bmj.com/content/363/bmj.k4354
Iacobucci Gareth. Cannabis based drugs: GPs will have to manage patients’ expectations, says expert
BMJ 2018; 363 :k4354
In the most recent issue of the BMJ. The bit I’m most concerned about is the following:
“Some GPs will have specialist knowledge of cannabis based products and of their benefits in managing different conditions and will be confident in prescribing, Hamilton said. To GPs with less experience he recommends
discussing the issue with other members of the practice team and consulting
Google Scholar.”
No mention of librarians in there at all! They even pose the question:
“Hamilton said that guidance from the National Institute for Health and Clinical Excellence on the use of cannabis based products for medicinal use was not due until October 2019 and that the absence of an evidence
base could place extra pressure on doctors.
“Where do GPs and even the specialists look for evidence around how to make a clinical decision?” he asked. “It just isn’t there at the moment. The chances are the patients are more likely to be better informed about this than the practitioners.””
Apologies if this has already been discussed or isn’t of interest to you.
Thanks,
Sarah
