Whatever it is, perhaps it was released a bit too prematurely?

I won't throw stones about typos and spelling issues if I think about on projects that I've worked extensively on. I could blame my poor eyesight though:)

However, you would think that the social media links would have been checked. Facebook points to a Carescribe profile that is devoid of information, pointing to another website that is not active. Twitter points to the default Twitter.com view.

I did wonder how the investigation could take place. I am not based in the UK, however, so do not know if the GP's task is that of gathering paperwork from other specialists. That can be the case here, so a GP writing an assessment for some benefit or support may "just" take in reports from other accredited professionals who they probably sought a referral to.

Irrespective of the service, I would be very concerned about information security and handling (now speaking generally).
