Subject: Do you recognise this request?

Dear All,

We have received a sample for Erythropoietin measurement with no location / source lab.

No request form was received with the sample but there was a large sticker accompanying the sample.

The sample number is (we think) *4891234*

And the patient’s hospital number starts with the letters ‘HEY’.

The test code for Erythropoietin on the large sticker we received with the sample is ‘LEPRO

If you recognise this, please get in touch.

Kind Regards,

Carys Lippiatt
