 Thank you, James, for your comments and contributions! I cannot find any record of me having proposed the phrase or idea of disaster risk creation or of the contrast with DRR. All material points to you...
I did find the sentence "City and municipal governments can thus be key players in disaster risk creation and amplification or in disaster risk reduction." on page 143 of the World Disasters Report 2010. The phrase "disaster risk creation" seems to be used coincidentally rather than with any deep meaning or analysis. 2010 comes after  "The Creation of Cultures of Risk" which James mentions below.
JC Gaillard https://jcgaillard.wordpress.com kindly sent me a scan of "A manual for the implementation of disaster risk reduction measures" http://saudepublica.bvs.br/pesquisa/resource/pt/des-633 The article's text seems to use "disaster risk reduction", "risk reduction", and "disaster mitigation" synonymously. Specific examples are given, but no direct definitions. The article concludes that "The manual is expected to be published in January 1989". I cannot find a manual with this title online.

 September 2008 is the date of my unpublished effort with the title: "The Creation of Cultures of Risk." With examples from 15 countries, its content and thinking became absorbed in preparation of "The Good, the Bad and the Ugly", finally of 2012. It did take a long time I do recall.
 
 I think this must have been the reason for Ilan's very characteristic declination of credit but, unless Ilan's records show otherwise, I am currently sure it was Ilan who came up with the neatly condensed "Disaster Risk Creation versus Disaster Risk Reduction".
