Dear Mailbase I'd be interested to find out what cut-off other labs have for doing a direct ISE in lipaemic samples to avoid reporting falsely low sodiums, and whether this is based on the lipaemic index or triglyceride concentration. Previously we didn't have indices so relied on visual inspection and measured sodium by direct ISE for any samples that looked lipaemic. We now have indices set up on our new Roche analysers - the kit insert for ISEs states that there's no interference up to an L index of 2000 but notes that there's a poor correlation between the L index (measure of turbidity) and the triglyceride concentration.
