Essie the advice from Diana Kloss is that it is not a legal document in all senses.    It is a statement of eligibility for SSP and that is all.   However you are correct in stating that it would be wrong for another clinician to alter the note.
 
  
 
In the same vein we would not alter our clinical notes or reports just because the client asks us to.  We would only make significant changes if there was a factual inaccuracy or not disclose a diagnosis that the client wasn’t happy to disclose to the manager, but that would be done at the time of report writing and gaining consent.    As Diana said, the client can say withhold the sick note from the manager (it shouldn’t be going to OH in the first place) or say to the manager that they disagree with what the doctor has written but they should take that up with the GP at the time of writing.
