We also have the Sysmex XS-1000i with auto mixer sited in our Emergency Department.  The POCT team performs maintenance and ensures stock is adequate.  The ED staff process the samples.

We have pretty much the same pros and cons as Deborah but have one exception that orders are placed for FBC on the electronic patient record and barcoded stickers are generated with unique 'accession / lab number' generated.  These are placed on the FBC sample and samples are run on the analyser after being scanned into WinPath Enterprise by the ED staff.  This means all results will go back to WinPath Enterprise and then to the electronic patient record.

The ED love having the analyser and have stated that they cannot do without it.  We very rarely have issues with it and have 50 - 100 samples per day depending on patient volume into the ED.  All grades of staff use the analyser - clinicians, nurses and HCAs.

Results correlate very well with the Haematology's Sysmex XN analysers and with UKNEQAS EQA.  We have the same issue with only 20 passwords so have a record book next to the analyser where users are supposed to record when they test samples on the analyser.  There is also a log on the electronic patient record of who ordered a test e.g. FBC (this is usually the person who tests the sample but not always which is why we have the record book).

It is important to note that when either of the EPR or WinPath IT systems are 'down' we ask that samples are sent to Haematology as the IT system connected to WinPath Enterprise is complex.  It is better for the sample to be logged into WinPath by the labs and then processed by Haematology and tested on the XNs so that we can ensure that the results will then post onto the Patient's electronic record.

We use a couple of Sysmex XS-1000i with mixer unit in our Oncology clinics.  Daily check, reagent changes, maintenance IQC and EQA etc are performed by Pathology Lab staff during out of hours 20:00 - 08:00 (takes an average of 30 min per analyser).  Correlation is excellent with main lab analysers (Sysmex XNs).  UKNEQASH compliance/reports are also fine.

Nursing staff perform the routine sample analysis during clinic 08:00-20:00.  It is easy to use.

IT side is more complex - the POCT and Main lab analysers all transmit results through a single Sysmex interface to the pathology computer database.  From there they are transmitted to our hospital results server where they collate into the patient records.  We do not allow results to be printed directly from analyser as we could not get the POC analyser to host-query with our PMI to add the patient details on the printout - an anonymous printout was unacceptable, hence the results must be accessed from the hospital results database.

Analyser also only has room for 20 unique passwords - so we cannot password protect the analyser from untrained users.  The analyser also does not automatically log-off so once a user is logged in the analyser remains open to all users.
