What operating system are you using? If you are using OSX, type this into a terminal:

defaults delete uk.fmrib.ox.ac.fslview

You are on linux, there will be a file somewhere in your home directory - on CentOS7, type this:

rm ~/.qt/fslviewrc

I'm not sure about other linux distributions.
