I’ve been asked to try to find examples of any breaches / prosecutions / fines, under either:-

Health and safety at work act

Corporate manslaughter act 2007

With respect to:-

Lone working

Violence against staff

I’m struggling to find anything so far, and will submit FOI requests to both the Crown Prosecution Service and NHSBSA (for NHS Protect), but just wondered if anyone had any examples to hand or good ideas of how / where to find this information.
