As ever, I would guess that the initial affine registration went wrong. Try CheckReg of the original image and one of the MNI-space images released with SPM. If the misalignment is greater than about 3cm or 15 degrees, you may need to manually reposition the scan. Alternatively, the affine registration part of the SPM12 segmentation is a bit more robust.
