This is great, thanks I am just trying to re do our health questionnaire. Does anyone please have a healthy survey, I'd like to capture the health status/awareness /belief of our employees to further measure and then construct appropriate targeted health awareness campaigns.

Would anyone be willing to share their pre placement health questionnaire, which covers specific exposures but is laid out as such  that the applicant would only need complete the relevant sections which were applicable to their specific job and therefore avoid unnecessary form filling . For example  an office worker would only  need to complete 3 or 4 general questions  but an applicant working with chemicals or at heights would be required to answer several sections.?

Any advice would be really helpful
