We launched our online form in September and went for the tick box option, the user has to tick the box to be allowed to submit.

Hi,
We are just moving over to an electronic request form and wondered how other libraries deal with their copyright declarations.
Do you use a check box (I Agree) and have a separate submit button?
or
Do you have a statement next to the submit button (by clicking on the submit button you agree to the copyright declaration).
Any advice would be greatly appreciated.
