You can use the command Glm (or Glm_gui in the Mac) to create this or any other design. See some examples in the manual, in particular this one: http://fsl.fmrib.ox.ac.uk/fsl/fslwiki/GLM#Two-Group_Difference_Adjusted_for_Covariate
