What is the purpose? If it's to respond to complaints then it would be needed in F2F too and seems a lot of work and cost for a very minor situations that can already be managed by other processes  (I always document if any whiff of conflict in my notes). If it's instead of clinical notes then great in theory but personally I'd need to be able to see trancript to help me formulate my report or would take paper notes anyway that would then need to be shredded.

They had to use specific types of stick for the recordings that met security standards - at the time we used kingston data locker plus but this has been superseded. As a side note if you use Mac kingston sticks are not yet compliant with El Capitan and waiting for a patch. 

They are a good way to transport confidential data and recognised by various data agencies for meeting certain encryption standards.

I've never experienced anything similar since thank goodness (I'm touching wood as I type!!)

Again as already mentioned, consent is important and a thorough explanation of the content of any report is essential before you release. I've been in OH for 14 years now and feel confident in assessing individuals and there are times when we dicuss them recieving the report prior to its release, even though I have gone through the content; sometimes of someone is new to OH,  they are a little guarded with a lack of understanding of our role and our position within an organisation. I always explain however that only content that is factually incorrect would be changed.
