You could explore the Integrated Services page on the Commissioning Handbook http://commissioning.libraryservices.nhs.uk/hot-topics/integrated-services - however I’ve noticed that a couple of the links are broken so apologies in advance.



Also you could try an advanced Google search using a domain search e.g. “integrated care pathway” site:nhs.uk, or the same on site: ccg.nhs.uk (with the space after the colon). Then play around with the keywords e.g. model, “service model” etc.

Subject: Models for integrated care pathways



Our Health Board is looking at models for integrated care pathways, particularly those that integrate referral between between mental health and physical health services and which reflect best practice in all areas of patient need.



Would any of you kind people out there know if there are models for care pathways within your organization that you would be willing to share, which deal with any type of co-morbid care planning for people, between physical and mental health services. That would be in either direction and at any level of care  (i.e. from physical healthcare services to mental health or vice versa; community-based or acute care etc.). Common examples might be liaison psychiatry, ambulance staff referral or dementia.



Or would you have a contact number for your ICP co-ordinator who might might be willing to share models of practice.
