Other issues: - coregistration / normalization went wrong (seems not to be the case as you probably have overlaid the fMRI results onto the subject-specific normalized anatomical volume) - (fast) head motion and associated intensity artefacts (you seem not to account for head motion at all in the design) - very liberal thresholds - inefficient design, issues with high-pass filter, correlated regressors (not sure what the forth and fifth regressors represent, the forth one seems to be "on" very often, the fifth only at the beginning and the end of a run)
