Yes we always report them even if it's just an alarm chime.  As already said, if another student complains we need to have covered our backs.
 
Had 4 so far this season.

Unbelievably we have never had a phone go off during an exam…………. Until this morning in OCR GCSE Maths L

 

Have found an incident report form ?  is there any other form that need to be completed???
