There is also a free book published by arthritis research called "clinical assessment of the musculoskeletal system" and can be downloaded from the website under the health care professionals portal. It is very thorough and I use the basics for all my musculoskeletal assessments as I often find when I carry out work for others their driver assessment forms have  neck and trunk movement assessed with no consideration of shoulder/arm/hand(reach and movement) or if the hips are sufficiently flexible they can step up...
