Keith This seems to be an increasingly common occurrence. Green's Operative Hand Surgery has been doing this since the last edition was published. Even worse, the 6-volume Plastic Surgery, edited by Neligan, has the historical section for each chapter and some of the references available online only! Having checked with the publisher, I have activated online access on behalf of the library. Only library staff have access to the user name and password. However, this means that library staff can provide the full list of references to individuals who request them. I made a note to check this when our copy arrived. Indeed, the full reference lists are online only, with only "selected" references appearing in print. I am trying to find out from the publisher whether the full lists are going to be freely available to everyone for ever.
