Alan Hi, we used the device magic package  (http://www.devicemagic.com/) on
tablets for this sort of thing in both museums and and commercial
organisations.

The system allows for all sorts of drop downs, check boxes, text fields,
drawings, photos, gps tags, calculated fields etc

it even  allows you to structure questions so the answer to one field
defines which questions you see next.

You predefine the forms and where they are going to go email, or to a
spreadsheet etc or both.. the form is installed on the pad and off you go .
If  there is no network connection the form is sent back to the server when
you do so the interview can be done anywhere with or without network.

well worth having a look, and quite cheap per device.

we've done a few installs for people and happy to give you any pointers if
you need any cheers
