Hi Subhasish:
Several years ago, we also found that they were no longer sold by PI, so we purchased the disks and built the actuators ourselves. We experimented with them and improved the design. Below is a link to our paper. You should be able to buy the piezo crystals from PI, part PTYY-0153 with fired silver electrodes on both faces.

We used to buy it from Physik Instrumente. But when I called them yesterday, they said the item is discontinued. Can anyone please let me know if there are other vendors of this product? Thank you !
