Dear all

I wonder if you could me please? Our heart failure team are utilising a telephone follow up clinic for the prevention of readmission.

And they are looking for data surrounding patient satisfaction, in line with patient experience. The audit team have advised that the tools are numerous.

I have conducted a literature search however I am not finding much

Has anyone else been asked to find anything similar? In any other area

Your help is much appreciated
