Mathew - the NHS Records Management: Code of Practice, part 2, says copyright declaration forms must be kept for 6 years and thereafter disposed of securely. But the understanding is that this relates to those forms where a signature was required. Hope this helps Regards Outi Outi Pickering Assistant Librarian Oxford Health NHS Foundation Trust Staff Library, Warneford Hospital Headington Oxford OX3 7JX Tel. 01865 738738 Email [log in to unmask]<mailto:[log in to unmask]> Library website: http://www.oxfordhealth.nhs.uk/library/ If you have not had a reply to your email within one working day, please contact [log in to unmask]<mailto:[log in to unmask]> From: UK medical/ health care library community / information workers [mailto:[log in to unmask]] On Behalf Of Mathew Stone Sent: 07 April 2014 13:14 To: [log in to unmask] Subject: For how long do we need to keep document supply request forms? Good afternoon fellow librarians Here at my library we have an archive of old document supply/inter-library loan request forms (with copyright declarations) dating back to 2007. They take up quite a lot of space that could be usefully used for other purposes. And of course it's not a good idea to keep personal data without good cause. So I was wondering if we really need to keep 7 years' worth of these forms. How long do you keep such records at your library? And does anybody know if there is any guidance on this matter?
