Thank you for all of your responses to my query.

Here is a summary of some of the ideas I was sent for those who were interested:

1. An example of a Limit for the USA that could be adapted for the UK:

NOT (Europe[mesh] OR asia[mesh] OR Africa[mesh] OR Australia[mesh] OR south America[mesh] NOT (north America[mesh] AND (Europe[mesh] OR asia[mesh] OR Africa[mesh] OR Australia[mesh] OR south America[mesh]))) 

2. Use the MeSH terms (or any relevant geographical subject heading terms) for the UK (exploded to include individual countries) then add in a free text search (united kingdom or UK or britain or england or scotland or wales or northern ireland or british or english or scottish or welsh or northern irish or NHS etc) then add in the author's institution search (similar to above using the IN field.)

1. MEDLINE; "United Kingdom".af OR UK.af OR "Great Britain".af OR England.af OR Scotland.af OR Wales.af OR Ireland.af; 3405647 results.
2. MEDLINE; "United Kingdom".in OR UK.in OR "Great Britain".in OR England.in OR Scotland.in OR Wales.in OR Ireland.in; 662639 results.
3. MEDLINE; 1 OR 2; 3405648 results.
