27 HERs said they did have guidelines or protocols for for groups/individuals who want to submit data to the HER when asked in 2012 Content and Computing Survey so there should be plenty of examples out there.

Also worth having a look at the BAJR guides perhaps? Looking at Guide 23 there appears to be a report template in there. Might be some other bits and pieces within other guides - http://www.bajr.org/BAJRread/BAJRGuides.asp
