I would ensure that the request is specifically for oh notes not "medical records" or company health files then you should send them..it is up to him who he lets have a copy of his oh notes.you should send them special delivery so you have a receipt on arrival to the person detailed on the consent. If it is to "bloggs,smith and brown solicitors" ask the chap for the name of an individual dealing with his claim.i always ask for this as I don't like sending to a business - it's a person in that business who has requested them. 
They should be used to oh particulars hopefully

Mrs hr may not be comfortable with his request but that is tough he wants what he is legally entitled to within the dpa. She will no doubt have to send her files too at some point!

Your posting suggested some concern that there was info on his mental health don't worry-he will be claiming for loss of earnings and personal injury which is likely to include some element of PTSD. Remember with civil claims it is up to the individual to prove the company did not do what it should ie - no risk assessment, he wasn't trained etc where the company legal team have to prove that they did!
look at it as cat and mouse.
He sounds like he is entitled to claim if he was doing what he should. ultimately if he has suffered ill health and loss if earnings the legal lot will decide what he should get.
As if he has worked for company A while off sick at company B he may not be entitled to as much loss if earnings/ company benefits etc.
Let them fight it out and see your chap as you normally would .
if you have concerns as to where notes are going etc given him a ring to let him know you have the request I usually say " you know how precious oh are about your medical info- I have a consent from---- and wanted to check before I send them---

I have a query I would appreciate your advice on. At one of the two companies I am shared with, there is a part time/as when required/available employee, (HGV driver), who was absent for a prolonged period of time last year, due to a work related accident at his full time role with another company.
 
I managed him back into work, on a phased return to work plan, and with some emotional support as he was struggling with returning to his full time role with the otther company, as he described/displayed what looked like a post traumatic reaction to the accident, (hit and seriously injured by reversing FLT at night -yes he was in the pedestrian area and wearing his hi vis) he presented as a sincere, genuine guy. I recommended if he was having these difficulties he discuss the possibility of some psychological support with his GP and company.
 
I have now receieved a signed consent form for my OH records from his other employers solicitors. He has put in a claim against this company. I am unsure of how to proceed with this.
1 - I have discussed this with my HR manager, and she has agreed to me seeking this advice from yourselves as she also does not feel comfortable with this request.
2 - I am not employed by the comapny he is claiming against.
3 - Am I leagally bound by this request and what do I disclose.
3 - Should I ask him what they are seeking and why.
4 - Should they not be seeking their own OH advice.
5 - Are there legal implications involved with disclosing his information to his other employer. (No request from his solicitors)
