Can someone tell us how to leave this list please? I certainly don't want to be part of a community where people feel it appropriate to tell tales to other people's supervisors.

Supervisors are supposed to be training their students and are ultimately responsible for what they do, therefore I would see it as wholly appropriate to contact them and alert them to training needs.

If this is an innocent mistake, which now looks like it's been dealt with. Is there really any need to cause further trouble/distress for the student by contacting their supervisor?

I agree.  If you want examples, then you can get excellent ones from the surveys posted on here so it's really not a big job to do.  I also think it's a data protection issue - that you need to tell people how their data will be stored, keep/destroyed.

Sending this to the list as well, because this is a discussion point at the very least.

That simply is not acceptable - you must have a brief and ensure your participants know their ethical rights (right to withdraw, confidentiality etc). You must tell participants that you have ethical approval.
Who is your supervisor(s)? Please send me their email address
