Without a doubt, qualtrics.

It lets you build as many blocks as you like with a free student account, it lets you control how many items are on a page, and it lets you see individual data (in excel form).

The customer help/guide is excellent and it also lets you measure reaction times.

I have also tried surveymonkey and surveygizmo.
