I would personally hand this one over to an OH Physician.. and if you haven't got one associated with the place of work , use an NHS OH or  another OH service that does.
If you have not got training in psychiatry, you give advice and something untoward happened you may have to explain your decision and why you thought you were competent to give it.
While I think my training and experience would allow me to give advice on mild to moderate mental health issues (stress anxiety mild depression etc).. this one is truly out of the ordinary, an abbatoir worker with cannibalistic thought. and feeling uncomfortable around knives ..
Even if you get reports from his specialist...I think making decisions to give advice about his fitness to work and modifications would be challenging. 
Let a physician take the risk... they get paid a lot more than an OHA ;)
There is no shame in referring onto an OHP... we have different but complementary skills...
