This is very true. DVLA recently sent out, in error by their warehouse, new D4 forms, on which the front page has to be completed by an Optician/Optrimist. We checked with the DVLA (and they recalled the forms, but there were a few hundred in circulation) and this is the new process and form about to be launched. So best advice is to hold fire and wait, as this new process is about to start. The Keystone does do the 6/7.5 and this is acceptable to the DVLA. Regards, Mike.
