This is a perfect demonstration of Reason’s Swiss cheese model of accident causation, and shows how dangerous antimicrobials can be when
held in place by an 18 year old paperweight.

Subject: ACB nearly destroyed my office
An Inflammatory statement you may think!
Not at all !!
I walked in to my sunlit office this morning only to a strong smell of burning!!!
My trusted ACB FOCUS 94 paperweight had chosen to focus the suns rays and ignited a reference range document !!!!
Beware gifts and trinkets from professional bodies !!!!!
Should I report this to the MHRA ?
BW John
