MEDLINE is part of PubMed; PubMed, however, contains a number of additional elements (as Ahmed kindly pointed out).
Here are PubMed's total citations as of today http://www.ncbi.nlm.nih.gov/pubmed?term=all[sb]
As Connie says, all citations destined for MEDLINE are indexed; if they aren't meant to be in MEDLINE, they may still have index terms (see PMID: http://www.ncbi.nlm.nih.gov/pubmed/22667080)

Keep in mind that the National Library of Medicine leases MEDLINE information to private vendors such as OVID Technologies, EBSCO and SilverPlatter. They create files such as MEDLINE 2007-2012 (including "in process" citations) which have not been indexed yet.
