You still use Form 10.  If you look on the back of the form, point 1 it explains what to do.

Dear All

 

Is there a different form for Special Consideration for in relationship to a group application?????

 

 

I know for individuals you can do it online now – but it says this can’t be done for a group

 

The only form I can find is JCQ/SC Form 10 – but that seems to be for an individual?

 

 

Any help would be appreciated
