Just came across this dated video which accompanied John Forrester's Effective Cycling - http://www.seidlerproductions.com/products/effective_cycling/ and scale down to vids.

The acceptance of Forrester's Vehicular Cycling (i.e. 'survival guide') mantra seems to be on the wane as some USA administrations recognise that mass participation in everyday cycling will not happen under current autocentric configured systems hostile to cycling. It seems six 'showcase' cities are ploughing ahead and dedicating protected space for cycling through the Green Lane Project. I'll be watching with interest.

Anybody in the USA on this group enlightened or have experience of this project and would like to comment?
