Hi Deb We have a formal complaints system where we might capture this information. However, we are also launching VOICE, an informal reporting system in September (we've had a system for some time, but it was rather ad hoc and not an "official" informal system, if that makes sense!). VOICE will be available to students, staff and the University community. I mentioned this a little while ago through this network and so just to keep all informed that expressed an interest, once the final leaflet and form has been approved and launched, happy to share this with those that contacted me and how we developed it.
