Not to derail the discussion or anything, but Justus's comment reminded 
me of an anecdote about EF Schumacher, the author of "Small is 
Beautiful".  He was reported as saying that if he had grown up in a 
society in which everything was run in small-scale enterprises, he would 
have written a book called "Big is Beautiful".

There are analogous reasons why EBHC focuses on the E part.  If we lived 
in an era in which everything was evaluated with unbiased systematic 
reviews and individual studies, perfectly reported and widely 
accessible, we might be talking about "Expert-based health care" instead.
