All our employees receive an Audio and audio questionnaire on entry and exit of the business. However the exit audio is not mandatory.Otherwise our exit health surveillance is based on risk assessment, and falls in line with whatever health surveillance they have had within their employment, kind of like a final reading on your utilities when you move house.
 
We simply update their health surveillance notes, and send a confirmation slip (duplicate) to HR to verify their exit health surveillance has been completed.

Good morning all

 

Does anyone have an exit medical questionnaire that they use and are happy to share with me. Have recently been asked to start conducting exit medicals by our insurers and they mention a questionnaire but are somewhat vague regarding the content.
